Transitional Care Program To Host Open House On January 23

GENEVA –A public open house will be held to showcase the newly renovated Transitional Care Program located at the Living Center at Geneva-South on Wednesday, January 23 from 4 – 5:30 p.m. with a ribbon cutting ceremony at 4:30 p.m. Tours of the unit will be conducted. Refreshments and door prizes will be provided.

The Transitional Care Program (TCP) is designed for individuals requiring short
term rehabilitation as part of their transition plan from the hospital to home.
TCP is intended to meet the rehabilitative and physical needs of individuals after
an acute hospitalization for planned and unplanned orthopedic procedures, as
well as extended medical hospitalizations. TCP recognizes that by optimizing the functional independence of individuals they will achieve greater outcomes.

The Transitional Care Program offers 24-hour care by a team of healthcare professionals made up of physicians, nurses, physical therapists, occupational therapists, speech language pathologists, social workers, certified nurses' assistants, activities and dietary personnel.
