DIABETES HEALTH SERVICES

Individual Diabetes Education & Diabetes Nutrition Counseling:  Available for: Adults, Children, Individuals with Gestational Diabetes, Individuals needing Insulin Pump Therapy Training and/or Continuous Glucose Monitoring (Personal & Professional).

Education sessions provided directly in your FLH Medical, P.C. office & also in the Independent Diabetes Health Services Office on the Geneva Campus. Call your FLH Medical, P.C. provider to meet with the Diabetes Educator. For providers other FLH Medical, P.C., call 315-787-4006.

Comprehensive Diabetes Education Course "Living With Diabetes":  A 7-hour course for adults with diabetes, their families, caregivers and support persons. Lunch and all class materials supplied at no charge. Pre-registration is required. Call Diabetes Health Services at 315-787-4006. Medicare insurance requires physician referral, which can be handled by the diabetes office. Program cost is covered by most insurances.

Diabetes Support Groups:  Free monthly (except December) meetings held at Geneva General Hospital and in Penn Yan at St. Mark's Senior Living Apartments.  Meetings are open to the public, walk-ins welcome - no registration required. Monthly films, , and recipe sharing. See Calendar of Events for date and times.

Diabetes Awareness Community Outreach:  Instructional staff is available for community and industrial work site group presentations upon request. Call 315-787-4006 for information or to schedule your event.
