Transitional Care Unit at Living Center at Geneva-South Receives Score of 100% Patient Satisfaction
Geneva, NY---The Transitional Care Unit (TCP) at the Living Center at Geneva-South is proud to have earned a score of 100% patient satisfaction, November – December, 2009. Of those surveyed, 100 percent of respondents gave top ratings for “Recommendation to Others” and “Overall Satisfaction.”

Some of the comments patients shared:

“Good care of patients and attitude. All meals are excellent and plenty. Nurses are concerned and considerate to all patients.”

“I am extremely pleased with the care, attitude, and quality of this facility. Would recommend.”

“(They) made me feel at home.”

The Transitional Care Program offers a comprehensive rehabilitation approach designed to improve the functional independence of patients who have had a hip or knee replacement, fractured bones, an amputation, a cardiovascular accident, pneumonia, congestive heart failure, heart ailments, or other diagnoses. Following care at TCP, 85 percent of patients return to their home environment, and others transfer to a different level of care.

The Transitional Care Program offers 24-hour care by a team of healthcare professionals comprised of physicians, nurse practitioners, nurses, physical therapists, occupational therapists, speech language pathologists, social workers, certified nurses’ assistants, activities and dietary personnel. The TCP is short-term placement designed to meet the rehabilitative and physical needs of patients/residents while they are “transitioning” to another level of care or returned home. The team of therapists provides rehabilitative services in the same building and is supervised by a board-certified, clinical specialist in geriatric physical therapy.

Admission to the Transitional Care Program can be coordinated by discussing the program by calling the admissions staff at (315) 787-4733.
