Community Service Plan
As part of our commitment to our communities, Finger Lakes Health partners with public health departments as well as other human services providers in Ontario, Seneca and Yates counties, using the framework of the New York State Health Commissioner’s “Prevention Agenda” as a basis for our community health planning. We work together to conduct a Community Health Needs Assessment (CHNA) every three years to determine the most pressing healthcare needs in our communities. The information is analyzed to rank top priorities and we then develop multi-year action plans to target those priorities.
