Diabetes Care Center
The Finger Lakes Health Diabetes Care Center provides a team of medical professionals with advanced education and experience in the field of diabetes, led by nurse practitioner Rhonda Solomon who has 15 years of experience and is board-certified in Advanced Diabetes Management. The philosophy of the center is based on the fact that excellent diabetic care is the results of a team of professionals working together. People with all types of diabetes and at every stage of the disease can expect to get the most up to date care in a kind and compassionate environment.
Rhonda Solomon, CDE, MSN, ADM-BC, FNP, BC provides experienced management for people with Prediabetes, Type 1 Diabetes , Type 2 Diabetes, Gestational diabetes (During Pregnancy), and weight loss. Management of diabetes is based on the current standards of care recommended by the American Diabetes Associated and the American Academy of Endocrinologists. Plans often include a combination of continuous glucose monitoring, oral medications, insulins, and pump management.
Cheryl Andreas, RN is the care coordinator for the center and assists patients getting to the appropriate professional, understanding medications, how to take them, insurance coverage, and cost of prescriptions. For more information, call (315) 787-4006.
The center has two diabetes educators:
Amanda Tourtellotte, BS, RDN, CDN, CDE, registered dietician, facilitates the “Living With Diabetes” Program, support groups and individual counseling for carb counting and diabetes self care based on accredidated standards.
Patricia Cochrane, BS, RN, CDE ,registered nurse, supports the insulin pump program and offers pump classes for individuals and groups, Sensor technology and continuous glucose monitoring.
Outpatient dietitian Christina H. Ganzon, RDN, CDN provides individual nutritional counseling along with group programs including the Diabetes Prevention Program. The goal of nutritional counseling is to improve lifestyle habits to manage or prevent chronic diseases such as Diabetes, Cardiovascular disease, Crohn's disease along with food intolerances, allergies and general weight loss. Consultations include nutrition education along with individual meal patterns and ideas. Nutrition counseling may or may not be covered by your insurance and often varies based on your diagnosis and insurance. Nutritional Counseling is located within the Diabetes Care Center at 293 West North Street in Geneva.
