Community Resources
Every six years, New York State requires local health departments and hospital systems to collaborate on a joint Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). These plans align with the New York State Department of Health (NYSDOH) Prevention Agenda, as well as the standards set by the Public Health Accreditation Board (PHAB).
As part of this process, local partners select at least three priority areas from the Prevention Agenda to guide community health improvement efforts.
