Finger Lakes Health's Seneca Family Health Center is located at 367A East Main Street, Waterloo, New York 13165. Dr. Luke Chen, Dr. Michael Kozlowski, Dr. Eric Shives, Jamie Barber, AGPCNP-BC, Christina Gray, MSN, ANP, CNS, Susan Randall-Mantella, MSN, FNP-C, Elizabeth Smith, MSN, FNP-C and Jennifer Whitmore, MSN, AGPCNP-BC practice family medicine at this site, providing both adult and pediatric care.

We’re happy to share that we are currently accepting new patients. Our friendly team is here to make joining our practice easy — we can guide you through the enrollment process, which many insurance carriers require.

We accept most health insurance plans, as well as cash, checks, and major credit cards. To help serve you best, we encourage scheduling an appointment in advance.

For your convenience, we also offer Telemedicine visits — a simple and secure way to meet with your provider from home. While Telemedicine works well for many health concerns, some conditions may still require an in-person visit.

The office is open Monday - Thursday from 7:30 a.m. to 5:00 p.m. and Fridays from 7:30 a.m. to 4:00 p.m. The primary office phone number for the Seneca Family Health Center is (315) 787-4977.

Registered patients, can visit our patient portal. If you are not registered for the patient portal, please contact the practice for activation instructions. You can send a message to your provider, request an appointment, request a prescription refill and more on the patient portal. All patients need to contact the practice for a new login.
